Dipping my toes into 2E and I think I have a basic understanding of the rules except for one thing: Does your opponent have free reign to complete missions if you have run out of cards in your dilemma deck? Or am I missing something?

Yes, if you run out of dilemmas your opponent can attempt their missions without facing any dilemmas. Most constructed decks will use more than the minimum 20 dilemmas to avoid that scenario.

This is basically Terok Nor's "thing"

One of the reasons I haven't run a dilemma pile under 40 in quite awhile.

This is one of the reasons I think the Terok Nor upgrade was Phase II's biggest success. It gave Terok Nor a unique identity tied to their subaffiliation, and it addressed a meta issue of over-proliferation of slim piles, especially ACE/TT targeted kill piles.

I don't disagree, but non-cheese combo piles were the collateral damage.

Take some real balls to run Tillman these days, though Hot Mess does make it at least a little bit more tempting.

It peaked in 2005, and fell off after Necessary Execution changed the way people did mission attempts. But man it had a hell of a run in between.

The definition for Tillman that I work off of is a dilemma pile that can put two damage markers on a ship in a single attempt. By that definition, you've not only seen one, you've played one.
