Starting to get deja vu here...

(I have never owned a Decipher tournament foil in my life, and I also don't have a scanner, or I'd help you out here.)

This means there are 6 Mendons:
- 1994 Black Border
- 1994 White Border
- 1994 Silver Border
- 1995 White Border
- 1995 White Border from the 2-player game
- 2000 Black Border from the tribble game

Is that the most number of physical variants? (Medical Kit also has 6 physical variants: BB, 94, 95, Silver, Tribble, Voyager)

More than likely, the WB95 version with "the" in the second line is from the actual WB95 set and the WB95 reprint version with "the" in the third line is from the 2-player game.

The WB95 set version is an errata version of the WB94 set where specialist (from the WB94 set) has been corrected to specialists,

Another version of the WB95 version with "the" in the third line is the card from the Cut "Uncut" set where cards from the three print sheets of WB95 (one common, one uncommon, and one rare sheets) were cut to form a set with the Official Uncut Seal Collectable imprint on the reverse side of the card. Only about 1000 of these sets were made.
