A few cards allow "exchange" with cards in your discard pile. Obviously, there are the new Kirk's reading glasses, but there's also Inspection.

When such an exchange is performed, does the card that is exchanged into my discard pile end up in precisely the same position in the discard pile as the card it was exchanged for, or does it go to the top of the discard pile?

Obviously, position of the discard pile matters for things like Process Ore as well as for things like Nanoprobe Resuscitation and Fal-tor-pan.

The entry you're looking for is discard pile:
When you play a card which allows you to "exchange" it for a card in your discard pile (e.g., Palor Toff - Alien Trader, Res-Q), the two cards trade places: the card from the discard pile goes into your hand, and the card you played takes the position of that card in the discard pile.
