It's been a long time since I have seen the episode, but why doesn't Lon Suder, Casual Killer have Telepathy?

It's suggested, but never outright confirmed, that this is at least partially why he has sociopathic tendencies. Basically, he's unable to see other people as real.
This interpretation also relies on the TNG episode "The Loss", in which Troi loses her Empathy and proclaims at one point that without it, even Riker feels like an empty projection with no more reality to her than a Holodeck character.

"Most Betazoids can sense other people's emotions. I can't even sense my own."
